The patient presented with a history of nasal fracture. She noticed deviation of her nose to the right. She also had difficulty breathing through her nose. An open rhinoplasty approach was performed with reduction of the small nasal hump, osteotomies, placement of spreader grafts, septoplasty, and nasal tip refinement.

The patient presented with nasal airway obstruction and cosmetic concerns with the shape of her nose. The patient requested a more refined nasal dorsum and nasal tip. Dr. David Bray, Jr. performed nasal airway reconstruction and cosmetic rhinoplasty.

The patient has a history of nasal trauma from water polo. He had nasal airway obstruction and a deviated caudal septum. Dr. Bray, Jr. performed nasal septoplasty with caudal septal relocation. The photograph demonstrates improved airway on the patients right after the septum was relocated to the midline.

In this case the junction of the lateral crura and accessory cartilage caused collapse into the airway. A lateral crural strut graft was placed. This patient achieved additional airway improvement from septoplasty and spreader grafts. The columella was straightened with a columellar strut. The medial crural footplate was reduced with suture placement and fixation toward the midline.

Patient presented with nasal airway obstruction. She had a previous rhinoplasty performed by a different surgeon over 20 years ago. On exam she had an inverted V deformity and collapse of her internal nasal valve. Dr. Bray, Jr., performed an open rhinoplasty with placement of spreader grafts, and a diced cartilage dorsal graft. The patient had improved breathing and aesthetic appearance.

This patient sustained a severe nasal and septal fracture. The nasal trauma resulted in nasal airway obstruction and nasal deformity. Dr. Bray, Jr. performed open septorhinoplasty to improve the airway and straighten the nose. The nostril view shows improvement of the columella achieved by caudal septal relocation and columellar strut placement. An alar contour graft was placed on the left to support the nostril. Spreader grafts and osteotomies were also performed.

The patient presented with a history of multiple nasal fractures. He also reported previous nasal surgery performed in Greece 25 years ago. The patient was concerned with severe nasal airway obstruction and nasal deviation. Dr. Bray, Jr. performed a complex revision septorhinoplasty with temporalis fascia graft and polydioxanone plate to improve the nasal airway and straighten the nose. This surgery required an open approach with multiple cartilage grafts from the nasal septum. Multiple osteotomies were performed to reposition the nasal bones toward the midline. The patient reports significant airway improvement following his rhinoplasty.

The presented for cosmetic rhinoplasty. The patient was primarily concerned with nasal deviation, nasal dorsal hump, and a long appearing nose. Dr. David Bray, Jr. performed a rhinoplasty with nasal dorsal hump reduction, caudal septal reduction, repositioning of the nasal septum toward the midline, osteotomies, spreader grafts, nasal tip suturing, and alar contour grafts.

The patient presented with a history of previous rhinoplasty performed by a different surgeon. The patient was concerned with her profile. Dr. David Bray Jr. performed revision rhinoplasty to improve the contour of the nasal dorsum and to improve projection and rotation of nasal tip.

The patient presented with concerns regarding the shape of her nose after undergoing two previous cosmetic rhinoplasties. The patient’s primary concerns included dorsal irregularity, wide nasal bones, and nasal valve collapse. Dr. David Bray, Jr. performed an open rhinoplasty with correction of the dorsal irregularity, osteotomies to improve the width of the nasal bones, spreader grafts to improve nasal valve collapse, and a temporalis fascia graft over the nasal dorsum to further improve the contour.

The patient presented with a history of previous rhinoplasty performed by a different surgeon. She was concerned with nasal airway obstruction as well as the appearance of the nose. Dr. David Bray, Jr. performed nasal airway reconstruction with placement of spreader grafts to the internal nasal valve and lateral crural strut grafts to the nostril rims. Dr. Bray, Jr. also improved the appearance of the nose. A temporalis fascia graft was placed over the dorsum to improve the dorsal irregularity. The alar(nostril) rim was lowered to a more aesthetic position using a lateral crural strut graft as can be seen on the lateral view. The nasal tip was improved by reducing the projection and improving the symmetry of the nasal tip cartilages. The patient was happy with the overall improvement in the appearance of her nose as well as the improvement in her nasal airway.
